It was very difficult. Submitted an article, came back in 20 days, the reviewers had few opinions and suggested acceptance, but the editor said it was not quite suitable for its readers and rejected it. Submitted the same article to another journal with an impact factor of around 7 without making any changes, and it was accepted after minor revisions in about two months. CMAJ is very strict with an acceptance rate of less than 5%, but the impact factor is decreasing every year... so I really do not recommend submitting to CMAJ
